Tbilisi City Hall is implementing a “Home Care” program for citizens in need and is currently co-financing services for more than 600 citizens at their homes. Such a statement made today Mayor of the capital.
“The program includes complex measures to manage citizens’ health, personal care, assist with daily activities, and provide psychosocial support to their loved ones.
Based on an individual plan, assistance is provided at the beneficiary’s home by a multidisciplinary team.
The program is available to individuals registered in the unified database of socially vulnerable families with a rating score of 200,000 or less, who, according to a special physical capability assessment questionnaire, have no more than 95 points on the Barthel Index.
To enroll in the program, citizens or their authorized representatives must call the Tbilisi City Hall hotline - 2 72 22 22”, - said Kakha Kaladze.
